Project HeadStrong requires certain assessments on the even number months. Depending on what is going on with you different assessments are required.
The assessments are:
Generalized Anxiety Disorder (version 7) or the GAD-7. It has 7 questions and it measures anxiety.
Patient Health Questionnaire (version 9) or the PHQ-9. It has 10 questions and it measures depression.
National Institute of Health, General Life Satisfaction or the NIH-GLS. It has 5 questions and measure life satisfaction.
Post Traumatic Growth Inventory or the PGI - It has 10 questions and it measures whether people see benefits, including changes in perceptions of self, relationships with others, and philosophy of life, based on the work to recover from trauma.
Posttraumatic Stress Disorder Checklist (version 5) or the PCL-5 The PTSD Checklist for DSM-5. It is a 20-item self-report measure that assesses the presence and severity of PTSD symptoms
Insomnia Severity Index or the ISI. It has 7 questions and it is designed to assess the nature, severity, and impact of insomnia and monitor treatment response in adults.
